Другой не менее важный с практической точки зрения аспект этой проблемы связан с сочетанным применением НПВП и ацетилсалициловой кислоты. Очевидно, что необходимость такой терапии может быть весьма высока, учитывая пожилой возраст пациентов, являющихся основными «потребителями» НПВП, и высокий риск кардиоваскулярных катастроф у пациентов с воспалительными ревматическими заболеваниями. Поскольку сам по себе прием низких доз ацетилсалициловой кислоты может вызывать развитие тяжелых осложнений со стороны ЖКТ [56], возникает закономерный вопрос, каковы реальные преимущества ингибиторов ЦОГ-2 перед «стандартными» НПВП у пациентов, вынужденных принимать низкие дозы ацетилсалициловой кислоты [57,58]. Действительно, по данным исследования CLASS достоверное уменьшение частоты тяжелых гастроэнтерологических побочных эффектов на фоне лечения целекоксибом (по сравнению с «неселективными» НПВП) выявлено только у пациентов, не получавших низкие дозы ацетилсалициловой кислоты [59]. Однако недавний мета-анализ результатов испытаний целекоксиба свидетельствует о четкой тенденции к снижению частоты как симптоматических побочных эффектов, так и тяжелых осложнений со стороны ЖКТ на фоне лечения ингибиторами ЦОГ-2 по сравнению со «стандартными» НПВП. Частота тяжелых гастроэнтерологических осложнений у пациентов, получавших низкие дозы ацетилсалициловой кислоты, была на 51% меньше на фоне приема целекоксиба, чем НПВП [60].
При выборе НПВП необходимо учитывать, что некоторые из них (например, ибупрофен и индометацин) обладают способностью отменять «антитромботический» эффект низких доз ацетилсалициловой кислоты [61], в то время как другие (кетопрофен, диклофенак), а также «селективные» ингибиторы ЦОГ-2 не проявляют этот эффект [62-64]. Совсем недавно было установлено, что на фоне приема ибупрофена наблюдается увеличение риска кардиоваскулярных катастроф по сравнению с приемом других НПВП [65]. Таким образом, пациентам, имеющим кардиоваскулярные факторы риска, на фоне приема НПВП (независимо от их ЦОГ селективности) необходимо назначать низкие дозы ацетилсалициловой кислоты [20,35]. Наиболее оптимальными препаратами у пациентов, принимающих низкие дозы ацетилсалициловой кислоты, вероятно, являются ингибиторы ЦОГ-2 [20].
Патология легких
Примерно у 10-20% пациентов, страдающих бронхиальной астмой, наблюдается гиперчувствительность к ацетилсалициловой кислоте и НПВП, проявляющаяся тяжелым обострением астмы. Эта патология ранее называлась «аспирин чувствительная бронхиальная астма, а в настоящее время «аспирин-индуцированное респираторное заболевание» (aspirin exacerbated respiratory disease) [66]. Установлено, что ингибиторы ЦОГ-2 (нимесулид, мелоксикам, целекоксиб, рофекоксиб) не обладают перекрестной реактивностью с ацетилсалициловой кислотой и НПВП в отношении индукции обострения астмы [66] и являются препаратами «выбора» у этой категории пациентов.
Репарация переломов
В недавних исследованиях было установлено, что «стандартные» НПВП и ингибиторы ЦОГ-2 в равной степени оказывают негативное влияние на консолидацию переломов у лабораторных животных. Это привлекло внимание к проблеме рациональной анальгезии и пациентов с переломами костей скелета, в том числе остеопоретических. Клинические данные, касающиеся влияния НПВП на срастание переломов костей скелета крайне немногочисленны. Предварительные результаты свидетельствуют о негативном влиянии «стандартных» НПВП на заживление переломов позвоночника и об отсутствии такового у ингибиторов ЦОГ-2. До получения более доказательных данных все же следует рекомендовать по возможности ограничить применение НПВП для анальгезии у пациентов с переломами костей [67].
В заключение необходимо подчеркнуть, что лечение НПВП продолжает оставаться трудным разделом фармакотерапии заболеваний человека. Появление ингибиторов ЦОГ-2, с одной стороны, сделало лечение более безопасным, с другой стороны - привлекло внимание к ряду новых аспектов противовоспалительной и анальгетической терапии НПВП (табл. 4). Мы надеемся, что представленные данные позволят врачам оказывать более квалифицированную помощь пациентам с болями различной природы и избежать ошибок, которые могут привести к нежелательным последствиям для здоровья и даже жизни пациентов.
